Green Chimneys is Hiring a Care Manager (Health Homes Program) Near Brewster, NY

The Care Manager is ultimately responsible for the overall provision and coordination of services to assigned caseload. He/she guides program enrollees and their caretakers (legal guardians) through the health care system by assisting with access issues, developing relationships with service providers and tracking interventions and outcomes. Is responsible for assisting the family in identifying its strengths and needs, while also providing counseling and crisis intervention services to youth and families as needed. This professional is instrumental in the care of their clients by leading and encouraging them.

Education/Experience:

  • Minimum of an Associate’s Degree with two years’ experience is required
    Bachelors of Arts or Science or Registered Nurse or Licensed Practical Nurse with two years’ experience is preferred. Candidates that can demonstrate they are working toward a Bachelor’s degree with a graduation date within one year can be considered.
  • Experience providing service coordination and information, linkages, and referrals for community-based services.
  • Relevant expertise and experience in serving children and families in child welfare, developmental disabilities, mental health, healthcare, and/or other systems, as well as those receiving preventive services.
  • Demonstrated understanding of the needs of high acuity children and their families as evidenced by additional years of experience, education, and training.

    Licensure/Certification:

    Valid driver’s license and a registered operating car to be used for daily transportation for work related purposes

    Completion of the OMH approved training in Individualized Care Model

    NYS license in field of degree if applicable

    *** Applicant must be bilingual Spanish
    Pay Rate: $38,000-$45,000 per year
